What is Leukemia?

Leukemia is a type of cancer in blood cells. Leukemia originates in the bone marrow, where blood cells are made. This blood cancer produces an uncontrollable amount of abnormal white blood cells, called Leukemia cells or blasts. The exact cause of Leukemia is unknown. A combination of genetic conditions and environmental factors are believed to be risk factors.

The current medical treatments given usually are a combination of chemotherapy, radiotherapy, and targeted therapy. Certain types of Leukemia can be managed by waiting. In 2015, 2.3 million people worldwide had Leukemia and caused 353,500 mortality case. This disease is the most common type of cancer in children, with 75% of cases in children being the Acute Lymphoblastic (ALL) type.

R. Virchow coined the term “Leukemia” in 1851 from Greek (leukos, means clear or white) and haima (blood). This disease was also formerly called Leucemia.

General Classification

4 Leukemia Types

There are 2 types of Leukemia, Acute and Chronic forms:

1. Acute Leukemia

This type is characterized by a rapid increase in the amount of immature blood cells. The overload of immature blood cells cause blood marrow to not be able to produce healthy blood cells, resulting in low platelets and hemoglobin.

2. Chronic Leukemia

The chronic type is characterized by the excessive buildup of relatively mature, albeit abnormal white blood cells. This disease may take months or years to progress, causing overload of abnormal white blood cells.

Leukemia is also divided into the kind of blood cells affected, namely Lymphoblastic and Myeloid Leukemia:

Lymphoblastic

The cancerous change usually takes place in a type of marrow cells that normally form lymphocytes, which are immune system cells that fight infection. Most Lymphocytic Leukemia happens in the B cell.

Myeloid

In this type of cancer, the cancerous process involves in marrow cells that usually form red blood cells, some types of white cells and platelets.

Pra-leukemia

Transient Myeloproliferative Disease, also known as Transient Leukemia, involves the abnormal buildup of non-cancerous megakaryoblasts. This condition usually happens in individuals with Down syndrome, or genetic changes similar to Down syndrome. Most of the time, this condition resolves within 3 months.

TXL Phytoceutical’s Research on Leukemia

Abstract: Tien-Hsien Liquid (THL, Taiwanese name version of TXL) is a Chinese herbal mixture that has been used worldwide as complementary treatment for cancer patients in the past decade. Recently, THL has been shown to induce apoptosis in various types of solid tumor cells in vitro.

However, the underlying molecular mechanisms have not yet been well elucidated. In this study, we explored the effects of THL on acute promyelocytic leukemia (APL) NB4 cells, which could be effectively treated by some traditional Chinese remedies. The results showed THL could induce G2/M arrest and apoptosis in NB4 cells.

Accordingly, the decrease of cyclin A and B1 were observed in THL-treated cells. The THL-induced apoptosis was accompanied with caspase-3 activation and decrease of PML-RARα fusion protein. Moreover, DNA methyltransferase 1 and oncogenic signaling pathways such as Akt/mTOR, Stat3 and ERK were also down-regulated by THL.

By using ethyl acetate extraction and silica gel chromatography, an active fraction of THL named as EAS5 was isolated. At about 0.5-1% of the dose of THL, EAS5 appeared to have most of THL-induced multiple molecular targeting effects in NB4 cells. Based on the findings of these multi-targeting effects, THL might be regarding as a complementary and alternative therapeutic agent for refractory APL.

In 2011, the research of TXL towards Acute Promyelocytic Leukemia (APL) by Chih-Jung Yao’s research team in Taipei’s Medical University, Cancer Center, was published in Hindawi’s Evidence-Based Complementary and Alternative Medicine Journal. The research primarily investigates the effects of TXL on the human NB4 APL cutured cells and the related molecular mechanisms.

Results of the Research

1. TXL Induced Apoptosis (Programmed Cell Death) of Leukemia Cells

Cell viability was determined by trypan blue exclusion. The control group is treated with phosphate buffered saline (PBS). The efficacy of TXL is expressed as a percentage of the control group.

Figure 1 demonstrates that TXL dose-dependently decreases the viability of Leukemia cells. When treated with 0.375 mg/mL TXL for 72 hours, only 51.49% of Leukemia cells survived, compared with control group. As the TXL’s concentration increases, only 6.71% Leukemia cells survived.

This research uses flow-cytometry to detect sub-G1 DNA content, which measures Leukemia cells that undergo apoptosis due to lost of DNA. Therefore the higher is the sub-G1 percentage/population, the higher is the percentage of Leukemia cells that undergo programmed cell death.

After 72 hours, when the dose of TXL is higher than 0.75 mg/mL, more than 60% of Leukemia cells undergo apoptosis compared to 3.13% of control group. This demonstrates that at higher dose, TXL works more effectively to trigger apoptosis in Leukemia cells.

2. TXL Induces G2/M Phase Arrest, thus Inhibiting Growth of Leukemia CellsG2/M

Normal and Leukemia cells both undergo mitosis (cell-division) for growth. There is a cell-cycle checkpoint between G2 and M(itosis) before cells initiate mitosis. Cells with damaged DNA or incompletely replicated DNA will instead undergo apoptosis or death, instead of going to division/growth process.

Therefore, the G2/M phase arrest refers to the stopping of mitosis at between G2 and M cell division process

In the research, it is demonstrated that TXL induces/triggers G2/M arrest in Leukemia (NB4 sample) cells. The cell-cycle distribution among the control group (PBS), TXL/THL, and ATRA (all-trans retinoic acid, an agent commonly used to treat APL (Acute Promyelocytic Leukemia) is measured for 24 hours and 48 hours.

Table 1 shows that at dose of 1.5mg/mL, TXL increased the percentage of cells in G2/M in a time-dependent manner. After 24 and 48 hours, it is evident that the percentage of G2/M phase arrest population of TXL (50.28% and 55.93%) are markedly higher than the control PBS population (19.21% and 23.27%).

3. TXL Phytoceutical Down-regulated PML-RARα Fusion Protein and DNMT1

At the dose above 0.75 mg/mL, TXL almost completely diminished the PML-RARα Fusion Protein. This result coincides with the remarkable apoptosis induction shows earlier. PML-RARα Fusion Protein cooperates with DNMT1 to exert oncogenic (cancerous) effects.

After 72 hours, TXL at the dose of 0.75 mg/mL also significantly down-regulated DNMT1. The conclusion is that TXL Phytoceutical substantially decreased both PML-RARα Fusion Protein & DNMT1 protein of Leukemia cells.

4. TXL Inhibits Oncogenic/Cancer Signaling Pathways

It is reported that the proliferation and apoptotic (programmed cell death) resistance of Leukemia cells are closely related to several Oncogenic signaling pathways such as Akt/mTOR, Stat3 and ERK. The above Western Blot Analysis in figure 4 demonstrates that TXL significantly inhibited phosphorylated Akt and mTOR. Phosphorylation refers to the introduction of phosphoryl (PO3) group into a molecule.

After 72 hours of treatment of TXL towards Leukemia cells, Akt protein that is related to Leukemia proliferation was decreased in a dose-dependent manner. TXL decreased both AKT oncogenic pathway activity and total AKT protein. Similar results are also seen in the mTOR oncogenic pathway.

This incident happened two years ago. In June, an incident occurred. Although there are no symptoms such as pain, contrary to the hope that tomorrow would be fine, I did a blood test at a clinic near my house. For normal women, leukocytes should be in the range of 4,000-8,000, my leukocyte test results were 150 million units!! I was immediately referred to the Main Hospital and given blood medicine. I was diagnosed with Acute Myelogenous Leukemia (AML) right away and was hospitalized.

My husband and my eldest son who accompanied me were shocked to hear about leukemia. I had intravenous drip chemotherapy every 5-6 days for 5 times.

During chemotherapy treatment, my appetite disappeared, I experienced side effects such as weakness, constipation, and mild nausea. During my fourth and fifth chemo, even as I lay in bed, I was constantly plagued by the feeling of not knowing what to do. I made it out of the hospital in mid-December and was able to leave the hospital but felt too weak to walk around the house.

And finally at the beginning of May last year I was able to leave the house alone and I am happy that my physical condition has recovered. But while taking a shower, I found a blue spot on my left thigh. I felt that the cancer must have recurred.

I was overwhelmed by hopelessness such that I had never experienced in my life, that night I believed the cancer had returned. The hospital examination showed a leukocyte level of 38,000 and according to the procedure I was immediately hospitalized.

My body was crying like it was holding back the side effects of the first chemo after being hospitalized again. I had severe stomatitis, my tongue was swollen and it looked like an allergy, even drinking water didn’t feel like drinking water anymore. Even when I just lied down, my facial muscles felt like they were being pulled and I felt intense pain.

Seeing that I was suffering from the side effects of chemo, my husband rushed to the library looking for books on cancer, he saw a book written by Professor Hiroyuki Abe of Kuroku Clinic and immediately gave me Tian Xian Liquid. Every day in June, I drank up to 4 bottles per day for 3 seasons.

Then, I realized that the side effects of June’s second chemo drug were quite mild. It was surprising that my colleagues in the hospital room, they said “I look fine now”.
